Previous Version of eScreen Paper COCs No Longer Accepted

As previously shared, beginning October 1, 2025, a new eScreen version of paper Chain of Custody forms (COCs) will be required.

Additionally, effective 01/01/2026:

- Quest DOT COCs will no longer be accepted
- Any COCs referencing the previous MRO are invalid

If you currently use paper COCs, you should have received a courtesy box of updated forms. If you have not received this, please contact our Support team to avoid disruptions.
